Disturbance of the Coagulatory System in Patients with Cerebrovasular Accident at Acute Stage



Abstract
It is well known that abnormalities of coagulation and fibrinolysis frequently take place during the course of cerebrovascular accident(CVA).
The authors analyzed coagulation and fibrinolytic studies in 80 patients at acute stage of CVA; 50 patients of intracerebral hemorrhage(ICH) and 30 patients of subarachnoid hemorrhage (SAH).
Tested items were platelet counts, prothrombin time (PT), activated partial thrombin time(APTT), fibrinogen and fibrin/fibrinogen degradation product (FDP). Blood were sampled. from peripheral vein within 48 hours from their onset. From the data collected in the present study, following results were obtained;
1) Decrease of platelet counts were observed in 6 patients and the prolongation of the coagulation time(PT, APTT) were seen in 7 cases.
2) Of all patients, the level of fibrinogen was kept in average of 298mg/dl and decrease of fibrinogen (below 199mg/dl was seen in 4 patients.
3) Average values of FDP in all patients were slightly increased (12.2¡¾4.4mg/ml in SAH, 15.9¡¾10.5,mg/ml in ICH) and each group of increased FDP above 10mg/ml were 62% in ICH and 30% in SAH.
4) Four patients(ICH: 3, SAH: 1) died of DIC following acute hemorrhagic onset.
Above results suggested that the evaluation of coagulation and fibrinolytic function at acute stage of CVA is not only important to know the occurrence of DIC but also useful to predict the severity and prognosis of cerebrovascular accident.
